Ships managed by the Synergy Marine Group — the company that managed the vessel that hit Baltimore's Key Bridge — have been involved in at least three deadly incidents since 2018, according to investigations and statements from officials in Australia, Singapore, and the Philippines. In 2018, a person onboard a vessel managed by Synergy in Australia was killed in an accident involving the ship’s personnel elevator, according to a report from the Australian Transportation Safety Bureau. “Many of these accidents involve the failure to apply existing safety management procedures and/or identified safety barriers that have proven effective in reducing the risks associated with elevator maintenance," the safety board's director, Stuart Macleod, was quoted saying in the report. In 2019, an officer on a Synergy-registered vessel in Singapore was reported missing after “likely fallen overboard while performing inspection or cleaning jobs at the outboard side,” according to a report by Singapore’s Transport Safety Investigation Bureau. The tanker referenced in the Philippine Coast Guard report, Petite Soeur, has been managed by Synergy since October 2022, according to the Electronic Quality Shipping Information System (Equasis) — a global tool promoting maritime safety.

WASHINGTON, Jan 24 (Reuters) - The National Transportation Safety Board (NTSB) on Tuesday faulted Ethiopia's final report into the March 2019 Boeing 737 MAX fatal crash and said investigators did not adequately address the performance of the flight crew. NTSB chair Jennifer Homendy said in an interview that Ethiopia's Aircraft Investigation Bureau (EAIB) had made errors in its report. "It's unprecedented -- under ICAO we get a right to review the report and to provide comment," Homendy said. The NTSB said the Ethiopian report's finding that aircraft electrical problems caused erroneous AOA output was "unsupported by evidence." The NTSB added that the Ethiopia report's finding that MCAS documentation for flight crews was "misleading since Boeing had provided the information to all 737 MAX operators four months before the Ethiopian Airlines crash."

Thai police arrested a man over an alleged fraud involving a firm that owned a New Jersey deli. Peter Coker Jr. was detained on January 11 at a hotel in Phuket, the Bangkok Post reported. His father, Peter Coker Sr., and James Patten were arrested in September and have pleaded not guilty. Coker Jr was the subject of arrest warrants issued by Interpol, per the Post, after being accused of stock market manipulation and fraud in September. The Post reported that Coker Jr. admitted he was the man wanted on the Interpol register.

Boeing has previously said the MCAS was a safety feature and the issues identified after the crash of flight 302, which followed one of a similar plane in Indonesia five months earlier, have been rectified. "Discussion of crew resource management and performance were still not sufficiently developed in the draft final report," the NTSB said. The accident involving Flight 302 followed another incident five months earlier, when the same model crashed in Indonesia, killing 189 people.
